      Description

      Curana Health is a provider of value-based primary care services for the senior living industry, including skilled nursing facilities, assisted & independent living communities, Memory Care units, and affordable senior housing sites.       Description

      The Patient Navigator role plays a crucial part in achieving Curana Health’s mission to improve the health, happiness, and dignity of our senior living residents. The Patient Navigator is crucial in ensuring a positive patient experience by managing incoming calls and providing exceptional customer service.

      Job Duties and Expectations

      Patient Interaction

      • Answer and manage incoming calls in a professional and courteous manner.
      • Greet patients warmly and assist them with inquiries or direct them to the appropriate department.

      Patient Assessment

      • Conduct a thorough assessment of patients’ health concerns over the phone.
      • Gather relevant medical history, symptoms, and other pertinent information to determine the urgency and nature of the medical issue.
      • Determine the appropriate level of care needed
        and guide patients to the most suitable healthcare resources.

      Communication

      • Relay messages and information between patients and healthcare teams
      • Collaborate with other staff members to ensure seamless communication within our health network.

      Qualifications

      • High School Diploma or equivalent
      • Exceptional customer service skills
      • Organized and effective communicator
      • Ability to maintain confidentiality and handle sensitive information.
      • Proficient in using computer systems and electronic health records.
      • Strong problem-solving skills
      • Previous experience in a medical office or healthcare setting is preferred.
      • Team player with a positive attitude
