- High School diploma or equivalent
- Proven solid computer skills, proficient in Microsoft Office

The Patient Navigator is responsible for initial triage of members, and the administrative intake of patients. Includes managing incoming calls, managing requests for services from providers/members, providing information on available network services and transferring members as appropriate to clinical staff. The navigator calls the patient, updates demographics, updates communication preferences, checks eligibility, works recalls, and pushes out electronic access to medical record.

Required Qualifications:
New York Residents Only: The hourly range for New York residents is $16.54 to $32.55 per hour.
