      Job summary

      Are you a Registered Nurse looking for a new challenge?

      Sciensus are currently looking for a Patient Transfer Coordinator to join our Early Supported Discharge service in East Surrey Hospital in Redhill. We are looking for someone who will be an on-site expert, taking charge for ensuring the smooth and timely discharge of patients. As a Patient Transfer Coordinator, you will take responsibility for assessing the suitability of patients to complete their inpatient pathway in their own home, supported by our filed team of clinicians. It is our aim to expedite the patients recovery, whilst maintaining the highest standard of care.

      Main duties of the job

      You will be responsible for meeting capacity and contractual targets in a timely manner, whilst always maintaining high degrees of safety and delivering the highest standard of clinical care. Completing clinical assessments of the patient in the ward and determining patients suitability for transfer home, compiling care plans and then transitioning care to the multi-disciplinary field team.

      Accurately documenting all patient care and clinical information to provide real time data and clinical history to ensure quality care is maintained and delivered. Maintain and demonstrate up to date knowledge, ensuring any gaps in clinical competency are addressed whilst also staying compliant with all business policies and procedures. Occasional visits to treat patients within their own home may be required.

      Managing the daily nursing capacity and case load of patients receiving care and ensuring that their expected service end dates are monitored and managed so that relevant discharge plans are put in place to ensure timely discharge back to community services.

      Proactively engaging and establishing exceptional relationships, internally and externally with both stakeholders and patients to improve performance and assist the growth of the business.
