What to Expect (Job Responsibilities):
- Facilitate timely and appropriate discharge and care transition support services
- Conduct functional needs assessments, develop service plans, and provide service coordination
- Maintain and distribute information about community resources
- Assist and support Medicaid consumers during the care transition process
- Participate in discharge planning, service coordination, and implementation of service plans
What is Required (Qualifications):
- Bachelor's degree in a related field or equivalent combination of education and experience
- Qualified Mental Health Associate (QMHA) Certification required
- 2+ years of clinical experience
- Reside in Oregon
- Travel required for the position
How to Stand Out (Preferred Qualifications):
- 1+ year of care coordination experience
