      What to Expect (Job Responsibilities):
      - Facilitate timely discharges and care transition support services through collaboration with providers and community service agencies
      - Conduct brief functional needs assessments, develop service plans, and provide service coordination
      - Assist and support Medicaid consumers during the care transition process
      - Coordinate initiatives, track progress, and maintain effective filing systems and data management
      - Receive and process requests for service from various sources and coordinate with the team

      What is Required (Qualifications):
      - Minimum of 2 years of experience in a health care or social services environment
      - Knowledge of community resources and services available for individuals with serious and persistent mental illness (SPMI)
      - Strong organizational skills and attention to detail
      - Excellent communication skills, both verbal and written
      - Proficiency in data entry and management systems

      How to Stand Out (Preferred Qualifications):
      - Experience working with Medicaid consumers
      - Familiarity with care transition processes and discharge planning
      - Background in behavioral health or mental health services
      - Ability to facilitate consumer and/or provider education sessions
