  We are seeking an experienced, self-motivated, hard-working, and patient focused healthcare Advanced Nurse Practitioner to join our PCN. The post holder is a caring, compassionate and committed, experienced Nurse who, acting within their professional boundaries, provides care for the presenting patient, from initial history taking, to clinical assessment, diagnosis, treatment and evaluation of care. They will work both autonomously and collaboratively with GPs and other primary care professionals within the PCN to provide care and treatment to patients with either long term conditions or undiagnosed undifferentiated diagnoses, support the delivery of policy and procedures, and provide nurse leadership as required.

  Main duties of the job

  Clinically, the focus of the role is to provide an acute home visiting service, assessing housebound patients presenting with acute illnesses and injuries, as well as performing long-term condition reviews for this subset of patients as required by the PCN and in accordance to training and competencies.

  Please note you must have a minimum 6 months of relevant UK experience.

  About us

  Aplos Health PCN was founded in 2019 and consists of four practices in the London Borough of Lewisham -Sydenham Green Group Practice, The Vale Medical Centre, Wells Park Practice and Woolstone Medical Centre. We provide care to a patient population of 50,000. In a little over a year and despite a global pandemic we have developed excellent working relationships between the practices. We have run educational events for patients, started an adolescent and contraceptive clinic and are now looking forward to developing further innovative pathways and services for our patients.

  Job Description:

  Assess, diagnose, plan, implement and evaluate treatment/interventions and care for housebound patients presenting with an undifferentiated diagnosis

  Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

  Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

  Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

  Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

  Promote and deliver evidence-based care for patients presenting with aural conditions

  Meet the needs of patients presenting for opportunistic wound care

  Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

  Undertake annual long term condition reviews as per the requirements of QoF. This will include completing the template available on the GP systems and any requirements of locally commissioned services.

  Accurately record clinical results on the practice clinical systems (EMIS)

  Carry out phlebotomy as required, ensuring specimens are recorded and ready for onward transportation.

  Implement and participate in vaccination and immunisation programmes

  Carry out administrative tasks for example, but not limited to, call and recall activities as directed by senior members of the Practice & PCN Management team.

  Familiarity with the clinical system E MIS

  Utilise EMIS to record all treatment and care, applying all appropriate templates and protocols relating to recording of patient data.

  Extraction of necessary data from clinical correspondence and input into Electronic Patient Record, i.e., GP2GP and Docman system

  Ensure full and accurate recording in the patients computerised records.
