Job summary
South Rural PCN has an exciting opportunity for a care co-ordinator to join team, to support our GP practice and assist in re-shaping primary care across our communities, on a part time basis for 15 hours per week, based at Constable Country Medical Practice - East Bergholt. This is a permanent role.
Working very closely with the practice and the multidisciplinary team (MDT) within the PCN this role is pivotal in ensuring all patients receive the best possible care and service.
If you are a skilled care coordinator with excellent person-centred planning experience, with local knowledge of key support and advice for our patient population, then we'd love to hear from you.
Main duties of the job
You will support the practice in coordinating all key activity including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centred, including supporting digital initiatives and includes responsibilities for the coordination of the patients journey through primary care.

The PCN provides services to 56,000 patients across rural Suffolk. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre (Boxford), The Holbrook and Shotley Surgery and Needham Market Country Practice.
