      Job summary

      Join Our Forward-Thinking PCN as a Clinical Pharmacist.

      We are a dynamic Primary Care Network (PCN) committed to delivering exceptional patient care through innovation and collaboration. Our multidisciplinary team values the unique contributions of clinical pharmacists in enhancing medication management and patient outcomes.

      There are 5 practices within our PCN, however, you will be working in a primary practice, whilst also collaborating with your PCN Pharmacy peers across the PCN.

      The candidate will report into the Lead Clinical Pharmacist.

      Main duties of the job

      As a Clinical Pharmacist within our PCN, you will:

      • Conduct structured medication reviews (SMRs) for patients with complex needs.
      • Manage long-term conditions such as hypertension, asthma, and diabetes.
      • Provide expert advice on medicines optimization and safety.
      • Support repeat prescription processes and medication reconciliation.
      • Collaborate with GPs, nurses, and other healthcare professionals to deliver integrated care.

      Who We're Looking For

      • Registered with the General Pharmaceutical Council (GPhC).
      • Primary Care Pharmacy Education Patchway (PCPEP) qualification
      • Independent Prescriber Qualified - desireable
      • Experience in primary care setting
      • Strong communication and interpersonal skills.
      • Commitment to continuous professional development.
