- Registered with the General Pharmaceutical Council as a Pharmacist in the UK
- Postgraduate Diploma or equivalent (or be working towards one) - desirable
- Independent prescribing qualification (or to work towards this in the future) - desirable
- A minimum of 2 years post-registration clinical experience
- Understanding of working effectively within multidisciplinary groups
- Demonstrable experience in clinical pharmacy practice covering a broad range of specialities
- Recognises the roles of other colleagues within the organisation and their role to patient care
- Demonstrates use of appropriate communication to gain the cooperation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. ICSs)
- Demonstrates ability to work as a member of a team
- Is able to recognise personal limitations and refer to more appropriate colleague(s) when necessary
- Demonstrable experience of co-ordinating and leading on clinical audit.
- Understanding the principles of medicines optimisation
- Good core therapeutic knowledge
- Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice
- Understands and demonstrates the characteristics of a role model to members in the team and/or service
- Demonstrates ability to conduct teaching and assessment effectively according to a learning plan with supervision from a more experienced colleague
- Demonstrates self-development through continuous professional development activity
- Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice
- An appreciation of the nature of GPs and general practices

Job summary

Knowledge and experience of the NHS
At least two years post clinical pharmacist qualification working experience
Ability to work across organisations to achieve shared aims for all partner practices.
Maintain appropriate documentation within the work portfolio of work underway with practices.

Communicate effectively the progress of work in hand across all key stakeholders.
