Job summary
An exciting opportunity has arisen within SWAN Primary Care Network (PCN) to recruit an Enhanced Nurse to play a central role in our drive to improve health outcomes and reduce inequalities across our neighbourhood. This role is ideal for a registered nurse with a passion for proactive, personalised care, collaborative working, and developing the future workforce.
Working across our networks practices and communities, you will provide clinical leadership and mentorship, contribute to population health initiatives, and support our Care Home & Frailty team. You will be a key part of our neighbourhood-based, multidisciplinary approach to healthcare transformation.

About us
SWAN Primary Care Network constitutes 7 GP practices, located in South Wigan Ashton North area, with a collective patient population of around 35,500. The member practices are Marus Bridge Practice, Medicentre, Bryn Cross Surgery, South Wigan Medical Practice, Winstanley Medical Centre, The Grange Practice and Kumar Family Practice.

Mentorship and Clinical Supervision
Mentor and supervise PCN trainee roles (e.g., trainee nurses, nursing associates, GP assistants)
Support their clinical development and professional growth
Coordinate learning opportunities including training, shadowing, and case-based discussions
Provide regular feedback, conduct appraisals and contribute to skill development
Promote a culture of learning, reflection, and continuous improvement
Care Home and Frailty Support
Provide comprehensive health assessments and care planning for frail and housebound patients
Participate in multidisciplinary ward rounds in care homes
Offer proactive support for palliative care, dementia, and complex long-term conditions
Support patients and carers in managing conditions and navigating services
Deliver opportunistic health promotion and wellbeing advice
Lead or contribute to the development of personalised care and support plans
Maintain effective communication with wider clinical teams and families
Infection Control, Governance & Policy Management
Act as PCN Infection Prevention and Control (IPC) Lead
Oversee IPC audits, training and policy compliance across the PCN
Develop, implement and review clinical protocols and policies
Lead and participate in clinical audits and quality improvement initiatives
Support shared learning following significant events or near misses
Respond to national or local policy changes affecting clinical delivery
