      Job summary

      An exciting opportunity awaits the right candidate to work within an extremely supportive clinical team based in Dorchester. Our team of nurses and paramedics are led by an experienced Advanced Nurse Practitioner in providing high quality and personalised care to the frail and older patients living in care homes across Mid Dorset.

      We are looking for maternity cover for an initial period of six months, which may be extended to one year, commencing in May 2025.

      The role is advertised as ideally full time, but we would consider part time for the perfect candidate.

      Main duties of the job

      The Frailty Team is an advanced practitioner-led team which delivers care to a caseload of frail, older patients living in care homes across the Mid-Dorset locality.

      The Frailty Paramedic will have a clinical role working autonomously with supervision and support to assess, manage and monitor the health and social care needs of patients with complex, long term health problems. The post holder will work collaboratively with the Frailty Team, Care Home Teams, GPs, Community Nursing Team, Rehabilitation Teams, Community and Acute hospitals.The service will be delivered to a defined patient population requiring case management of frailty within the community setting.

      Be competent at identifying at risk patients and adding them to the caseload

      Proactively monitor and manage an identified patient caseload of patients within care/residential homes.

      Empower patients to self-manage their care at appropriate stages and support them to cope with changes

      Work with patients and carers to develop and review personalised treatment, care and management plans.

      Provide advice and information to assist individuals to make lifestyle changes and to manage and monitor their frailty.

      Support and manage patients care at the end of their life.

      Ensure that patients are appropriately referred to other practitioners when it is in the best interest of the individual needing action, care or treatment.

      Ensure that help is requested from a suitably qualified and experienced healthcare professional to carry out any action or procedure that is beyond the limits of your competence.

      Work in partnership with secondary care and Community Hospitals to facilitate the planning of care and services on discharge for case-managed patients.

      Work in partnership with clinicians with specialist skills and knowledge to meet the needs of people with complex long-term conditions (including frailty).

      Actively work towards reducing hospital bed days and innovative new ways of working to provide more care in the community.

      Take all reasonable personal precautions necessary to avoid any potential health risks to colleagues, people receiving care and the public.

      Remain objective and have clear professional boundaries at all times with people in your care, their families and carers.

      Safeguard and promote the welfare of vulnerable adults, children and young people in all areas of work.
