      Job summary

      Are you a newly qualified GP (within the last two years, without a substantive post) looking for an exciting and supportive role to launch your career?

      Dartford MODEL Primary Care Network is seeking an enthusiastic and forward-thinking GP to join our team.

      This role will provide you with the opportunity to work across our 3 practices; The Wellcome Practice, Lowfield Medical Centre and Dr C J Shimmins & Partners, within Dartford, North Kent.

      Our practices are dedicated to supporting the growth of both trainees and experienced clinical staff, with two of our sites operating as GP training practices. We have successfully supported GPs through the New to Practice programme, and have had GPs complete various fellowship programmes.

      Our team of current GPs brings a variety of specialist interests, including womens health, minor surgery, and medical leadership.

      Main duties of the job

      As a GP within Dartford MODEL Primary Care Network, you will be required to work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice.

      Key responsibilities include:

      Provide a full range of medical services as defined in the core GMS contract, additional and enhanced services where appropriate.

      Conducting routine and urgent appointments, both face-to-face and via telephone.

      Prescribing according to local and national guidelines.

      Handling home visits for routine and emergency cases.

      Managing administrative tasks, such as letters, results, referrals, and emails.

      Making appropriate referrals and managing related admin.

      Checking and signing repeat prescriptions.

      Participating in on-call/duty sessions as per pro-rata hours.

      Supporting Extended Hours services, enhancing patient access.

      Collaborating with other health and social care providers to deliver integrated care.

      Maintaining accurate consultation records using the EMIS clinical syste

      Offering clinical support to colleagues, helping the team meet high standards in patient care and QOF/Enhanced Services targets.

      Dartford MODEL PCN consists of 3 practices; The Wellcome Practice, Lowfield Medical Centre and Dr C J Shimmins & Partners.

      All three practices are based in Dartford, North Kent and serve a patient population of over 34,000 people.

      Clinical responsibilities:

      The post-holder will be required to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

      Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

      Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

      Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health.

      Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.

      Recording clear and concise consultation notes

      Collecting data for audit purposes.

      Prescribing in accordance with local formulary and national guidelines

      The post-holder may be required to visit patients during surgery time in an emergency if clinically indicated.

      The post-holder may be required to support enhanced access sessions as per the needs of the PCN.

      Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

      Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

      Prioritise, organise and manage own workload in a manner that maintains and promotes quality, including participating in shared learning across the practice, PCN and wider organisation. Work as part of the team to seek feedback, continually improve the service and contribute to business planning.

      Take responsibility for own learning and performance through continuous professional development, including participating in supervision, acting as a positive role model and assessing own learning needs.

      The GP will, as an integral part of and in the course of carrying out his/her role, have access to confidential and sometimes sensitive information relating to patients, carers and family members, as well as similar information about colleagues. They may also have access to confidential information about the practices and the PCN.

      All such information, in whatever format and howsoever made available, must be treated with the strictest confidence. All such information held within and outside of the practice will be shared only as necessary, and in accordance with the practices confidentiality / data sharing policies, and in line with data protection legislation and the Freedom of Information Act.
