Job Description

- Respond to referrals and provide assessment, diagnosis and treatment as first point of contact by attending to patients, according to patients needs either in the GP Practices, patient homes, or care homes
- Support and continue the development of an on the day emergency patient service
- Accurately triage and prioritise patients via the telephone, demonstrating excellent history taking skills to elicit the history of an event/illness, including past medical and drug history
- Lead on care home support- creating care plans for patients when they arrive at care homes, being the first point of contact for care homes, and running weekly ward rounds with the care staff
- Attend the weekly Paramedic CPD and MDT meetings, actively participating where required
- Complete a thorough history taking of patients
- Recognise own limitations of practice and know when to request help from colleagues
- Recognise acutely unwell patients and how and when to refer on
- Communicate well with colleagues to develop treatment/referral plans for patients
- Review the effectiveness of treatments given, making changes where necessary and knowing when to refer on
- Perform and interpret ECGs; alongside other results as appropriate
- Perform investigatory procedures as required
- Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc
- Maintain accurate medical records
- Write referrals to secondary care when necessary
- Discuss and anticipate end of life care with patients and their families
- A HCPC recognised Paramedic qualification and in active practice status with the HCPC without restrictions on practice. Full UK driving license
- BSc in a training programme approved by the College of Paramedics Health & Care Professions Council (HCPC) registration able to operate at an advanced level of clinical practice Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11. Post-reg Advanced Practice MSc in a training programme approved by Health Education England. Non-medical prescribing qualification.
- Has completed 2 years of "consolidation of learning" as a "newly qualified paramedic" and a further 5+ years experience as a band 6 or 7 (or equivalent) paramedic.
- Working towards Advanced Clinical Practitioner status. Mentorship or supervisory skills training. Experience within General Practice. Experience working with SystmOne and/or EMIS systems.

Job summary
Falmouth and Penryn Primary Care Network (PCN) is recruiting one full-time Paramedic to join our established team of Paramedics. This is an exciting opportunity for a motivated and forward-thinking Paramedic to work in a clinical, patient facing role, with the opportunity for supported development.
Prior experience in primary care is preferred but we would still welcome applications from experienced Paramedics without primary care experience. When starting the role you will initially have a period of induction where you will be able to shadow the existing Paramedics in their roles, and other health professionals. Support will be provided the PCN Management team, and from the current team of five Paramedics. Several members of our Paramedic Team are working towards the Advanced Clinical Practitioner (ACP) apprenticeship supported by our PCN.
Each Paramedic has a main base surgery where they work from, however, everyone is expected to work from each of the four surgeries as demand requires. Currently the Paramedics work 37.5 hours over four days.
This particular role will be based at Westover Surgery. Westover is our smallest surgery however you'll have the opportunity to work alongside our other team members across our other surgeries.

Our PCN has a population of over patients 48,000 living in Falmouth, Penryn and the surrounding areas. There are four GP practices within the PCN, in close proximity of each other. We have an interesting and diverse population, with students, young families and the elderly.
Falmouth has been named as the best place to live in the Southwest and it was noted for its buzz, and thriving new industries as well as the sea views, beaches and marinas. Falmouth also has good schools, a community hospital, and the district general hospital in Truro, a short distance away.

Paramedics are used to assessing and managing varying degrees of risk of complex presentations of acute and chronic problems in the absence of full or ambiguous information. Working within General Practice encourages advanced thinking and decision making with more information and the support of GP colleagues. You will be supported to develop this skill.
Our Paramedics primarily carry out home visits, providing additional support for the duty doctor with clinic appointments, both telephone and face to face appointments and completing weekly care home ward rounds.
Each care home within our PCN is aligned to one practice. You are likely to have one care home at which you will be expected to complete a weekly ward round with. The weekly ward round is flexible, varying from checking in with the home on the telephone, to visiting in person every week and seeing some of the patients face to face.
This is a rewarding part of the role as it enables the care homes to build up a trusting relationship with the paramedic, and for the paramedic to get to know the residents and their health problems well, improving the care given to these residents.
On Wednesday mornings the team of PCN Paramedics attend an online Multidisciplinary Team (MDT) meeting alongside GPs, a Geriatrician, Community Nurses, Dementia workers, and care home staff to discuss complex cases.
The role will offer you the opportunity to build on your experience and skills by working within a supportive multi-disciplinary healthcare team.

Recognise and work within own competence and professional code of conduct as regulated by the HCPC and College of Paramedics.
Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
Work in partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
Administrative requirements:
Have excellent written English skills to ensure clinical notes are complete, accurate and legible to the lay person.
Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN.
Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.
Review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
