Team Select Home Care, supports the efforts of children and young adults who are medically fragile to live at home with their families and thrive in their communities. Our nurses and families have round the clock support from our experienced clinical team. We are currently seeking a Registered Nurse RN or Licensed Practical Nurse LPN for day, night or weekend shifts to care for a child at their home in Aurora, Naperville and Yorkville IL.

Responsibilities

• Patient assessments


• Administration of prescribed medication


• Care planning


• Communication with other members of the patient's multi-disciplinary team


• Emergency management


• Health promotion and teaching

Qualifications

• Current RN or LPN license in IL


• Nursing experience, preferred


• Maintains a current CPR certification
