      The Pediatric Respite Nurse provides a variety of in-home healthcare support to patients. During respite shifts, s/he provides patient care addressing symptom management ordering appropriate interventions as needed. The Respite Nurse provides an ongoing assessment of patient needs, completing home health visits ensuring compliance with re-certification for home health guidelines.

      Position Details

      • Location: Duval county area, Home Health
      • Compensation: $68,000 -$72,000 base salary, plus paid driving mileage; full benefits package including 23 days of PTO annual
      • Days/Hours: Monday - Friday 8:30am - 5:00pm; occasional weekend shift usually 1x month, rotation based on-call

      Job Description

      • Assesses and identifies patient and family member needs during initial enrollment contact and subsequent nursing and respite visits.
      • Typical case load involves around 10-15 patient visits a week
      • Follows plan of care as ordered by the physician, including any nursing interventions, medication administration, treatments, bathing, transferring, feeding, teaching, and/or death/discharge process.
      • Completes in-home patient care documentation specific to interventions performed and any medications administered throughout patient care. Also documents post-mortem care and destruction of medications at the time of death, if applicable.
      • Communicates with manager/director if any problems, complaints, concerns related to the patient/patient care/services.
      • Adheres to all infection control and patient confidentiality policies and complaint process to the Florida Department of Health.
      • Provides patient follow-up contact, scheduling, enrollments and 60-day nursing visits.
      • Provides ongoing education, advocacy, and empowerment to pediatric patients, parent/guardian, to maintain safe care with dignity, and focus on quality of life.
      • Acts as a liaison between Community Hospice Pediatric Care interdisciplinary team and other professionals involved in the patients’ care.
      • Stays up-to-date with working knowledge of all Community Hospice PedsCare programs and services. Programs include Perinatal, Hospice, Partners-in-Care, non-reimbursed palliative care programs, inpatient pain and palliative physician consult services, regulations, and reimbursement mechanisms.
      • Participates in IDG meetings as a contributing member of the team.
      • Ensures timely completion of all discipline-specific and organizational documentation according to Community Hospice policies and procedures.
      • Participates in program, organizational, community in-service programs or presentations, as appropriate.
      • Becomes dually employed with Community Hospice’s home health agency, Alivia Care, and performs home health responsibilities including education, admissions, obtaining physician orders, and care coordination for Alivia Care clients.
      • Cross trains in palliative and hospice care for coverage of palliative and hospice nurses.
      • Other duties as assigned.

      Education/Experience:
      Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:

      • Bachelor's Nursing degree preferred, two-year Associates or degree from a nursing program minimum
      • BLS Certification
      • RN state of Florida licensure
      • Two (2) years nursing experience minimum, with two recent years related to pediatric nursing

      Desired Knowledge & Skills

      • Principles and practices of pediatric nursing
      • Ability to complete accurate physical assessments
      • Can work within a framework of various patient home health environments across locations.
