Day/Eve Rotation (United States of America) The Pharmacy Post-Graduate Year 1 resident will complete a one-year clinical training program to build on their Doctor of Pharmacy (PharmD) education to develop clinical skills responsible for medication-related care of patients with a wide range of conditions. Residents are prepared as pharmacists for a clinical practice that integrates direct patient care, drug distribution, teaching and management. The residency program accomplishes this by utilizing the four competency areas and their associated education goals and objectives as required by the American Society of Health-system Pharmacists (ASHP) standards. These competencies areas include patient care, advancing practice and improving patient care, leadership and management, and teaching, education, and dissemination of knowledge.Shift:
ESSENTIAL FUNCTIONS
Upon completion of an orientation period, the resident rotates through various areas of the Department of Pharmacy and clinical practice working alongside a preceptor to:
1. Perform clinical review of medication orders from prescribers throughout the hospital including Emergency room, Critical Care unit, Pediatrics and Medical Surgical units. Intervene and provide recommendations to providers for drug dosing errors, drug interactions, allergies or drug contraindications. Suggest appropriate or alternate drug therapy to the clinical team when needed.
2. Responsible for oversight of verification, preparation, and distribution of medication orders. Responsible for ensuring medications contain the correct labeling and drug components. Responsible for oversight of Sterile, Non-Sterile and Hazardous compounds and ensures adherence to Pharmacy policies.
3. Responsible for reviewing patient medication profiles and monitoring clinical parameters to ensure the proper choice of drug, dosage form, dose, length of therapy and interaction potential of the prescribed drug. The resident will review and complete Pharmacy algorithms for antibiotic dosing and Pharmacy drug interchanges for medications not on Formulary.
4. Participate in Multidisciplinary rounds throughout the resident’s clinical rotations as appropriate. Maintain clinical relationship with providers. Review patient clinical records and makes appropriate recommendations when needed.
5. Responsible for maintaining a working knowledge of, and conforming to legal requirements concerning the distribution of drugs, as well as good working knowledge of recent developments in the field of pharmacy. Completes yearly competencies assigned by the hospital.
6. Responsible for the distribution, control and record keeping of all controlled substances. Participates in weekly inventories of controlled substances when needed.
7. Safety Awareness – Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment. Participates in safety forums and meetings when asked upon. Participates in quality improvement and safety longitudinally.
8. Provide education for the South Shore Health professional staff (i.e. pharmacists, physicians, nurses).
9. Participate in periodic reviews of therapeutic classes of drugs and present findings to the Pharmacy and Therapeutics Committee.
10. Prepare and preset to the appropriate committees’ evidence-based reviews of drugs requested for formulary addition.
11. Initiate and facilitate completion of drug use evaluations.
12. Develop, implement, and participate in a clinical research project over the course of the year.
JOB REQUIREMENTS
Minimum Education - Preferred
Must be a graduate or candidate for graduation of an ACPE accredited college or university with a PharmD degree
Required Licenses
Pharmacy Intern - Board of Registration in Pharmacy (Massachusetts) at start of employment
Pharmacist – Board of Registration in Pharmacy (Massachusetts) within 90 days of employment as outlined in the Pharmacy Residency manual.
