      • Schedule: Monday-Friday, 8:00AM-4:30PM, and one weekend every 6 weeks, 8:00AM-4:30PM
      • Hospital: Ascension St. Agnes
      • Location: Baltimore, MD
      • Salary: $111,820.80 - $167,731.20

      Actual compensation offered will vary based on factors including applicable licenses, certifications, education, experience, location, qualifications and comparison with associates in similar roles.

      The position is for Internal Medicine with a focus in Antimicrobial Stewardship

      Responsibilities

      Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.

      • Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
      • Recommend formulary alternatives when appropriate.
      • Compound and dispense medications for accurate delivery to patients.
      • Provide drug information and education to medical staff and patients.
      • Communicate with physicians and other clinicians as necessary to facilitate care.
      • Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
      • Assist with academic functions such as serving as a preceptor for pharmacy students.
      • May float to provide coverage of pharmacy needs.
      • Provide patient counseling when required.

      Requirements

      Licensure / Certification / Registration:

      • Pharmacist credentialed from the Maryland Board of Pharmacy obtained prior to hire date or job transfer date required.

      Education:

      • Bachelor's or advanced degree in Pharmacy required.

      Additional Preferences

      Education:

      • Graduate of an ACPE accredited College of Pharmacy
      • Doctor of Pharmacy or other advanced degree highly preferred

      Experience:

      • Completion of an ASHP accredited PGY-1 Pharmacy Residency required
      • Completion of an ASHP accredited PGY-2 Pharmacotherapy or Infectious Diseases Residency is preferred
      • Clinical pharmacy experience (2-4 years) in a hospital setting preferred (required if no PGY-2 residency)
      • Board of Pharmacy Specialists certification in Pharmacotherapy or Infectious Diseases preferred
      • Knowledge of contemporary hospital/clinical practice and service
      • Significant knowledge and expertise in advanced pharmacy practice in Pharmacotherapy and Infectious Diseases/Antimicrobial Stewardship.
