Review and interpret prescriptions using patient profiles to verify dosing accuracy and prevent adverse drug interactions.

Collaborate with physicians to clarify prescription details and make dose adjustments based on renal and hepatic function, patient age, and disease state.

Prepare medications by weighing, measuring, compounding, and mixing ingredients using pharmacy equipment such as balances, mortars, pestles, and graduates.

Dispense IV additives, irrigating solutions, and prepare chemotherapy and sterile admixtures using aseptic technique under a laminar airflow hood when necessary.

Monitor pharmacy inventory to remove expired or ineffective drugs and ensure timely reordering to maintain adequate stock levels.

Organize daily operations and oversee support personnel in the dispensing function.

Educate patients and healthcare staff on medication usage, availability, investigational drugs, and toxicology.

Conduct in-service training sessions to keep hospital staff informed about pharmacy procedures and drug information.

Bachelor of Science (B.S.) degree in Pharmacy required.

Current and valid State of Connecticut Pharmacist License/Registration required.

Strong clinical knowledge, attention to detail, and excellent communication skills.

Experience in hospital pharmacy preferred.
