Conduct medication reviews for patients, including those in residential and nursing homes.
Develop and implement pharmaceutical care plans to optimise prescribing practices.
Provide clinical supervision and leadership to the medicines management staff.
Assist in managing and monitoring prescribing budgets.
Deliver medicine awareness projects and produce patient information materials.
Support the implementation of rational, cost-effective prescribing in line with local and national objectives.
They must be able to work independently, using their own initiative, whilst also demonstrating excellent collaboration skills, contributing fully to team projects, quality improvement activities, and wider practice initiatives.
Development robust systems to manage medicines safely during transitions of care particularly from secondary care settings ensuring a seamless, safe, and effective transfer process.
- Completion of pre-registration year, with some experience in a hospital or similar environment.
- Experience of working as a clinical pharmacist.
- Previous experience in undertaking audit and research is desirable.
- GPhC registered Pharmacist.
- Up-to-date broad clinical knowledge.
- Good consultation skills/experience
- Excellent understanding of pharmacy/medicines standards, legal requirements, and guidelines.
- Membership of the Royal Pharmaceutical Society, working towards the faculty framework.
- Independent prescribing qualification.
- Strong verbal and written communication skills.
- Ability to manage professionally challenging conversations and situations.
- Self-motivated, proactive, and able to act on own initiative.
- Able to work calmly under pressure and deal with conflicting pressures and deadlines.
- Ability to demonstrate safe, effective, and efficient use of medicines.
- Ability to support the development of clinical pharmacy services in line with local and national objectives.
