      Responsibilities

      • Responsible for completion of required rotations, rotational objectives and projects.
      • Responsible for staffing in the LMC pharmacy department (min: every other weekend and one evening per month).
      • Responsible for collaboratively determining proper individualized drug therapy for patients when required.
      • Responsible for collaboratively determining design, monitoring and evaluation of drug therapy when required.
      • Responsible for providing recommendations for therapy alterations in order to improve patient outcomes and decrease hospital costs.
      • Responsible for proficiency in required documentation of alterations of drug therapy and patient interactions.
      • Review formulary and collaboratively make changes to LMC’s formulary.
      • Participate in preparation of clinical practice guidelines.
      • Respond to informational requests from patients and healthcare professionals in a complete and timely manner.
      • Responsible for providing assessment and counseling of medications upon discharged when requested.
      • Participate in monitoring of pharmacokinetics per LMC procedures.
      • Responsible for following safety policies, procedures and protocols; report patient safety concerns, medical errors and unnecessary risk for staff and patients. Residents shall encourage patients to be advocates for their own safety and care by reporting and asking questions when they feel necessary.
      • Responsible for maintaining licensure and required continuing education.
      • Responsible for performing other duties as assigned or requested.

      Qualifications

      Job Requirements

      • Graduate from an accredited college of pharmacy
      • Must be licensed in the state of New Mexico or obtain such licensure within 90 days of start date
      • Healthcare Provider CPR certified or must obtain such certification within 30 days of beginning date
      • Supervisory experience

      Preferred Job Requirements

      • PharmD with Pharmacotherapy Board Certification
      • Bilingual - English/Spanish
