Job Description

- Support medicines reconciliation and synchronisation, particularly for patients transferring between care settings
- Liaise with community pharmacies, hospitals, and specialist services to ensure safe and effective continuity of care
- Work in partnership with patients to support appropriate medicine usage and adherence
- Ensure Shared Care Protocols (SCPs) are followed, referring back to specialists where appropriate
- Support the prescription ordering and processing system, ensuring safety and efficiency in handling repeat prescriptions
- Manage clinical document workflow, including reviewing and processing discharge summaries, clinic letters, and hospital communications
- Assist with training reception/admin staff on medicines ordering, streaming, and prescription requests
- Build relationships across primary, secondary, and community care settings to strengthen integration of pharmacy services
