- Support the repeat prescription process, including reviewing medication requests and liaising with patients, prescribers, and community pharmacies.
- Perform medication reconciliation following hospital discharge, outpatient appointments, or care transitions.
- Manage and maintain accurate medication records and ensure patient medication lists are up to date.

Job summary
We are seeking a motivated and detail-orientedPharmacy Technicianto join our multidisciplinary team at Spirit Primary Care. Working under the supervision of clinical pharmacists and GPs, you will play a vital role in supporting safe, effective, and efficient prescribing and medicines management within the practice.
This is an excellent opportunity to work in a patient-facing, primary care environment, contributing to the optimisation of medicines use, supporting medication safety initiatives, and improving outcomes for patients.
This role will be based across our Leicestershire practices with travel expected to sites.
