Job Description

- Provide a personalised intermediate care service to adults in their own home within Airedale, Wharfedale and Craven areas. This also includes providing therapy support to patients in Castleberg Hospital.
- Perform Physiotherapy assessment of patients with a diverse range of presentations and complicated physical and psychological conditions, to provide a diagnosis, develop and deliver an individualised treatment programme in conjunction with the rest of the team.
- Provide a key role in working in partnership with medical colleagues, multidisciplinary teams (MDT) and carers to set realistic objectives and to help to facilitate goal and discharge planning to enable safe discharge from our service.
- Work to support the team in its delivery of interventions, to skill share with other professionals to support the full breadth of the service (competency training is offered to support this skill sharing method of delivering intervention).
- Degree or equivalent pre-registration award in Physiotherapy
- UK Registrations with the Health Care Professions Council
- Relevant qualification in a related subject and / or evidence of post registration training and development
- Member of the Chartered Society of Physiotherapy
- Member of Physiotherapy special interest groups/organisations
- Physiotherapy assessment and treatment skills
- Clinical reasoning skills
- Planning and organisational skills
- Experience of working in community settings
- Supervisory and leadership skills
- Recent and relevant undergraduate experience working in a variety of clinical areas and settings
- Undergraduate experience of multidisciplinary team working and working collaboratively with other professions and agencies
- Experience in use of IT applications e.g., SystmOne
- Recent post graduate experience with relevance to the job
- Community or in-patient rehabilitation experience
- Full UK driving Licence
- Access to your own transport for work purposes
