Job summary
An exciting opportunity has arisen for a motivated and enthusiastic Band 6 Physiotherapist to join Swansea Community Integrated Services-Therapies Team, within Swansea Bay University Health Board
The post holder will provide a high standard physiotherapy service to the specialist caseload across various sites within the SBUHB asthe service requires having satisfactorily completed a range of core rotations in a variety of specialties will manage a defined caseloadof clients with complex needs within this setting, using evidence based/client centred principles to assess, plan, implement andevaluate interventions, maintaining associated records

Main duties of the job
To recognise good personal time management, punctuality and consistent reliable attendance, plan and prioritise own client workloadto meet service and patient priorities. Readjusting plans as situations/changes arise.
To Formulate and deliver an individual physiotherapy treatment programme based on sound knowledge of evidence-based practice andtreatment options using clinical assessment, reasoning skills and knowledge of treatment skills.
Undertake all aspects of clinical duties as an autonomous practitioner responsible for the management of your own workload and caseload within a defined area of the service working without direct supervision. Plan and organise a number of complex activities/ intervention programmes which require the formulation / adjustment of plans
Follow the standards of practice for Physiotherapy and ensure that minimum requirements are met (CSP Health Board, departmentaland service specific). Implement policies, propose and contribute to changes in working practices/ procedures for own area.

The team provides same day response and reablement to support people to be discharged in a timely and safe manner from hospital, or to prevent an admission to hospital or permanent placement into a care home. You will be supported by an experienced MDT including physiotherapists, occupational therapists, speech & language therapists, dietitians, medicine management, domiciliary care assistants, social workers and nursing colleagues.
There are CPD opportunities, for undergrad and post- graduate education. As part of your role you will be expected to support student placements and provide supervision to less experienced colleagues.
The successful post-holder will be expected to work flexibly across Swansea Bay and participate in 7 day working as required.
The ability to travel in a timely manner across sites during the working day is required.
