      POSITION SUMMARY:

      Under the direction of the Integrated Behavioral Health (IBH) leadership team and in collaboration with the Health Equity Accelerator, the Population-based care IBH clinician works as a member of the Adult Primary Care and Family Medicine team. The role of the clinician is to work collaboratively with the primary care team to ensure that the behavioral health needs of the patients are being met. The clinician will respond to referrals from PCPs and utilize registry data to provide proactive outreach to patients who have or are at high risk for developing behavioral health conditions like depression and anxiety. The clinician will also facilitate individual and group interventions with patients to improve their self-management of common co-occurring chronic medical conditions like diabetes and hypertension that can be exacerbated by behavioral health conditions. The position is currently a hybrid of telemedicine and in person, which may change based on hospital expectations. The clinician will work closely with a team of other clinicians, psychologists and psychiatrists within the Primary Care clinic.

      ESSENTIAL RESPONSIBILITIES/DUTIES:

      CLINICAL RESPONSIBILITES:

      • The clinician will conduct psychosocial assessments and crisis interventions when needed, formulate behavioral health diagnoses and case conceptualizations, develop and implement effective short-term treatment plans, facilitate group interventions and collaborate with primary care team members.
      • The clinician will adhere to trauma-informed, evidence-based principles in providing clinical care to patients and families.
      • The clinician will present cases in team meetings with the consulting Psychiatrist and Psychologist and communicate team recommendations back to Primary Care Providers.
      • The clinician will monitor patient outcomes using evidence-based tools (e.g. PHQ-9) and adjust treatment as needed.

      OTHER RESPONSIBILITES:

      • Attendance at IBH and Psychiatry meetings and in-service education offerings through the Department of Psychiatry
      • Attendance at General Internal Medicine meetings under the direction of supervisor
      • Obtaining continuing education as necessary to maintain licensure
      • Utilizes hospital's behavioral standards as the basis for decision making and to facilitate the hospital's mission.
      • Follows established hospital infection control and safety procedures.
      • Maintains patient confidentiality according to hospital policy. Practices professional discretion when determining the inclusion of any patient information into medical record or department clinical record notes.
      • Documents each patient encounter in the medical record or the Department clinical record concurrently according to Department policies
      • Utilizes hospital's behavioral standards as the basis for decision making and to facilitate the hospital's mission.
      • Perform other duties as requested

      • Models the ability to set professional boundaries

      JOB DESCRIPTION CHARACTERISTICS

      EDUCATION:

      Requires Master’s Degree from an accredited school of Social Work or school of Mental Health Counseling or related field as determined by the Board of Registration of Allied Mental Health and Human Services Professions or National Certified Counselor (NCC) preparing for licensure as an LMHC or working towards completion of Masters of Social Work (MSW) and required to obtain LCSW within 6 months of hire date.

      CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

      Requires a Massachusetts license as a Licensed Certified Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Independent Clinical Social Worker (LICSW). For LCSW, can be obtained within 6 months of employment. For Pre-LMHC, can be National Certified Counselor (NCC) preparing for licensure as an LMHC

      EXPERIENCE:

      • Previous experience in Integrated Behavioral Health or Medical Setting preferred.
      • Requires level of knowledge of Social Work process and community resources and agencies in relation to health, illness, and disability, and social determinants of health
      • Interest in chronic medical conditions and holistic approaches to health and wellbeing preferred.

      KNOWLEDGE AND SKILLS

      • Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families
      • Cultural sensitivity and responsiveness humility with patients from diverse backgrounds and identities.
      • Bicultural/Bilingual competency a preferred.
      • Ability to work in a multidisciplinary team.
      • Computer literacy with ability to use standard word processing, spreadsheet programs and e-mail (i.e. MS Word, Excel, Outlook) and web browsers.
      • Must be comfortable in a fast paced and dynamic work environment (sense of humor is valued).
