      What to Expect (Job Responsibilities):
      - Provide administrative oversight and clinical supervision of the Population Health Nurses
      - Monitor and oversee the activities of the Nurse Team to ensure member needs are addressed appropriately
      - Identify member needs and escalate issues to the Nurse Manager or other appropriate staff
      - Review data to ensure performance measures and benchmarks are consistently met
      - Provide education and guidance to staff to maintain adherence to accreditation standards

      What is Required (Qualifications):
      - Three (3) years of experience in care management, case management, or care coordination for BH/MH/SU/IDD populations
      - Minimum of three (3) years as a Registered Nurse (RN)
      - Licensed in North Carolina by the State Board of Nursing as an RN
      - Must have a valid driver’s license and reside within North Carolina
      - Ability to travel within the catchment area as required

      How to Stand Out (Preferred Qualifications):
      - Experience with acute care discharge planning and/or utilization management
      - Experience coordinating physical healthcare needs
      - At least one (1) year of experience providing direct supervision of staff delivering BH/MH/SU and/or IDD services
