What to Expect (Job Responsibilities):
- Coordinate scheduling and administration of Long Acting Injectables (LAIs) for patients
- Conduct follow-up outreach for clients admitted or recently discharged from the hospital
- Perform telephone outreach to facilitate care gap outreach based on population health indicators
- Provide instruction and health information on self-care and health maintenance
- Ensure compliance with state and federal privacy regulations and maintain confidentiality

What is Required (Qualifications):
- Current RN license in the State of Oregon
- Successful completion of an accredited nursing program (AD, BS, or AA degree)
- At least one year of nursing experience in care management or population health management
- Must be a resident of the State of Oregon
- Ability to work independently and manage time effectively

How to Stand Out (Preferred Qualifications):
- Experience in telephonic patient care or population health management
- Knowledge of de-escalation methods or willingness to be trained in them
- Familiarity with computerized client data information systems
