What to Expect (Job Responsibilities):
- Develop, implement, and evaluate comprehensive patient care plans for chronic health conditions
- Assess patients' conditions and ensure continuity of care through documentation and communication with healthcare providers
- Facilitate referrals and manage care transitions, ensuring all patient needs are addressed
- Conduct individual counseling sessions on rehabilitation treatment and health maintenance
- Participate in quality improvement projects and present outcomes in group settings
What is Required (Qualifications):
- Bachelor’s degree in business, behavioral/social sciences, public health, or related population health field
- Minimum of three years of experience in a relevant field such as public health or social sciences
- Experience in sales and marketing, disease management, and working directly with healthcare providers is preferred
- Strong organizational skills and ability to manage multiple tasks in a fast-paced environment
- Proficiency in using computer software tools for data management and documentation
How to Stand Out (Preferred Qualifications):
- Background in Social Work or direct experience with physicians and advanced practice providers
- Experience in community health outreach or health education initiatives
- Familiarity with motivational interviewing and active listening skills
- Demonstrated ability to analyze data trends and verify information
