      Job summary

      The Dalston Practice | East London

      Practice Clinical Pharmacist

      We are looking for an enthusiastic, committed and experienced practice clinical pharmacist to join our clinical team and work with us in a friendly, well-established practice. The practice is flexible with working hours and both full time and part time applicants are welcome to apply.

      The post holder will be a registered pharmacist with significant clinical and quality improvement experience and ideally an independent prescriber. The practice is looking for an individual who is committed to delivering a high standard of care to patients and is passionate about clinical pharmacy. The candidate must be able to work effectively as part of a diverse practice team. Although prior experience in General Practice is highly desirable, pharmacists from other sectors will be considered for the position.

      Previous NHS general practice clinical pharmacy experience is highly desirable. Good working knowledge and proficiency with all Microsoft Office applications essential. EMIS Web knowledge desirable.

      Hours

      The successful applicant will need to work between 20 to 38 hours per week, Monday to Friday.

      Salary and benefits

      £48,000 - £55,000 full time equivalent, depending upon experience

      Potential for performance based bonuses

      Key duties and responsibilities:

      Conduct surgery and telephone consultations with patients with minor illnesses

      Patient-facing medication reviews of patients with polypharmacy especially for older people and people with multiple co-morbidities.

      Provide advice and support for patients with self-limiting conditions

      Reconcile medicines following patients discharge from hospital or clinic appointments

      Review medications for newly registering patients

      Practice as an independent prescriber working within scope of practice

      The practice's aim is to provide our patients with an excellent standard of patient-centred care, with clinicians and patients jointly involved in all aspects of planning and delivering care to meet patients' health and well-being needs.
