- Lead proactive, evidence-based nursing care for people living with long-term conditions, supporting self-management and preventing avoidable deterioration.
- Provide high-quality treatment room and preventative care services across the age range, guided by current best practice and professional standards.
- Offer clear health education and personalised advice, helping patients understand their conditions and treatment plans.
- Work closely with GPs, HCAs, pharmacists and wider MDT colleagues to coordinate safe, joined-up care.
- Use clinical systems to record accurately, monitor outcomes and contribute to quality improvement, safety audits and practice targets.
- Play an active role in safeguarding, infection prevention and maintaining a positive, patient-centred culture at the surgery.
