      Job summary

      An exciting opportunity has arisen for a Practice Nurse to join Chillington Health Centre. This is a part time, permanent position of 19 hours per week.

      The nursing team is made up of 1 Nurse Practitioner, 2 Healthcare Assistants and a GP Assistant.

      The position will be well suited to an individual who has previous experience in general practice, however, this is not essential. The individual should be enthusiastic about providing high-quality patient service with strong communication and administrative skills.

      Salary is dependent on experience.

      Main duties of the job

      To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The successful candidate must be comfortable with change, and able to work autonomously. A willingness to learn and develop their skills and undertake training relevant to their role is also essential, with an openness to support the wider nursing team.

      Job description

      Job responsibilities

      To work as part of the primary healthcare/nursing team with responsibility for ensuring the delivery of safe and effective nursing care to the whole practice population. The focus of the role is the delivery of evidence-based practice for patients with long term conditions and management and preventative nursing interventions and delivering this care to patients.

      Your duties will include but are not limited to the following:

      Undertake cytology

      Develop, implement and embed health promotion and well-being programmes

      Routine acute wound care

      Care of chronic wounds, pressure and leg ulcers

      Travel advice and immunisation following patient groups directions

      Family planning contraception

      Hormone Replacement Treatment monitoring

      ECGs both acute and routine as required

      Peak Flow Readings

      Phlebotomy, as and when needed

      Chronic Disease Management (Diabetes, Asthma, COPD, AF, CKD)

      Hypertension Management

      Routine adult and child immunisations following patient group directions

      Doppler testing and stocking fitting

      Chaperoning or assisting another clinician as requested

      Assisting clinicians with minor procedures if required

      Occasionally may be required to perform in absence of HCAs

      Prepare and send samples for lab testing

      Removal of Sutures/Staples

      Urinalysis for both acute and chronic conditions

      Action cytology results as appropriate
