      Job summary

      Bramley Village Health and Wellbeing Centre is looking to recruit a full time Practice Nurse to be based solely at Bramley Village Health and Wellbeing Centre, LS13 2BL

      The surgery is looking to restructure it's nursing team by building a fresh and enthusiastic team that is committed to working alongside our team of GPs, ANPs and Medical students to provide outstanding patient care.

      We are looking for applicants with accredited Chronic Disease Management (Respiratory or Diabetes) Qualifications to provide effective Long Term condition management at our Bramley site which serves 7500 patients. The organisation is also committed to providing and supporting staff development via internal and external training.

      Having previously operated a team that was required to travel across our three Leeds sites, the surgery is now looking to add greater structure and ensure greater continuity of care by basing a full time practice nurse at each of our sites.

      Main duties of the job

      The Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting. This role involves a variety of clinical tasks including chronic disease management, minor ailment treatment, immunisations, and health education. The Practice Nurse will work under the guidance and supervision of our team of advanced nurse practitioners and our organisational nursing director in order to contribute to the practice's goals by delivering high-quality patient care and participating in health promotion initiatives.

      The Practice Nurse is expected to perform clinical procedures and manage patient follow-ups, ensuring the delivery of care is in accordance with the latest clinical guidelines and within the scope of practice. This position requires excellent nursing skills, a knowledge of Infection Prevention Control, a patient-centred approach to care, and the ability to work effectively within a multi-disciplinary team to improve health outcomes.

      About us

      The Fuller and Forbes Healthcare Group is a primary care provider with practices currently in Leeds, Devon, Burnley and Gateshead. The Leeds based practices provide care to 17,000 patients across Leeds at locations in Bramley, Middleton and Cottingley. The surgery is made up of a diversified clinical team consisting of GP's, GP Registars, ANPs, ACPs, and a nursing team, with clinical supervision and mentorship by GPs to medical students.

      Chronic Disease Management:

      Specialised care: Organise and run specialised clinics relating to long term conditions management such as: asthma, COPD, Diabetes and other relevant nurse-led clinical areas.

      Care Protocols: Follow protocols for the management of chronic diseases and chronic disease registers such as diabetes, asthma, COPD, and cardiovascular disease.

      Patient Monitoring: Regularly review and monitor patient health status and adjust care plans as necessary.

      Immunisation and Vaccination:

      Vaccination Schedules: Administer vaccines according to national immunisation schedules.

      Patient Follow-Up: Ensure proper follow-up and record management for vaccinations and immunisations.

      Administrative and Record Keeping:

      Documentation: Maintain accurate and detailed patient records, ensuring confidentiality and compliance with GDPR.

      Data Management: Utilise SystmOne to enter patient data and retrieve patient information efficiently.

      Team Collaboration:

      Multi-disciplinary Teamwork: Work closely with GPs, Lead Nurses, and other healthcare professionals to ensure cohesive patient care and service delivery.
