      Job summary

      We are looking for a dedicated and compassionate Practice Nurse to join our experienced and friendly team, 2.5 days a week (22.5hrs p/w).

      x2 days at 9 hrs (Tuesday & Thursday)

      x1 4.5 hrs (Wednesday AM or PM)

      The ideal candidate will have some prior experience working within a GP practice, but expertise in all areas outlined is not essential we are happy to support your professional growth.

      Main duties of the job

      • Assist with the delivery of high-quality nursing care in a GP surgery setting as part of a Nursing Team.
      • Manage chronic disease clinics E.g. Diabetes, Asthma, COPD, & Hypertension etc.
      • Conduct health screenings, vaccinations, and other health promotion activities.
      • Managing women's health, such as conducting cytology screening & contraception assessments.
      • Provide wound care, minor injury treatment, and patient assessments.
      • Support patients with mental health and wellbeing concerns.
      • Offer advice and education to patients on lifestyle changes and disease management.
      • Work closely with GPs and other healthcare professionals to provide comprehensive care.
      • Maintain accurate and confidential patient records.
      • Flexibility in provision of care by recognising and prioritising health issues and intervening appropriately.
      • Support the Practice Team in the event of clinical emergencies.
      • Maintaining up to date evidence-based knowledge and help support the development of others in the team.

      Essential Criteria and Skills Required

      Essential

      • - Experience in primary care or general practice nursing is essential.
      • - Registered Nurse (RN) with a current NMC registration
      • - Diplomas in some of the following clinical skills: long term condition management (Diabetes, Asthma and/ or COPD), women's health (smear taker or contraception), health promotion and wound care.
      • - Punctuality and commitment to teamwork.
      • - Flexibility to work outside office hours, if required.
      • - Strong communication and interpersonal skills.
      • - A proactive, holistic, patient-centred approach to care.

      • Registered Nurse (RN) with a current NMC registration.

      • Attained Immunisation and Vaccination foundation course.
