      Job summary

      We are looking a registered nurse to join our existing team of experienced practice nurses. We are looking for someone with experience of working in accordance with the UKCC Code of Professional Conduct and working within primary care. They must have a demonstrable commitment to professional development. This role is to cover Maternity Leave for 12 months starting mid-August / September, however there may be some movement and can be discussed.

      Our new Practice Nurse can expect to work in an encouraging, supportive, forward thinking, experienced Practice environment that is continually working to improve the quality of healthcare for our patient population.

      Our Practice Nurse role would be expected to undertake a range of nursing assessments and provide appropriate care or treatment in conjunction with our clinical team and Practice Policies.

      Ideally, you'd be able to support and deliver a high level of quality care and service based on the needs of the patient population of which would include treatment, management plans, screening, preventative care, and patient education. In addition to this, you would also be expected to support the Practice in meeting its contractual obligations and delivery of enhanced service specifications.

      Main duties of the job

      To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

      Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

      The Practice Nurse will be responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

      Ashfields Primary Care Centre is an innovative and forward-thinking Practice led by our partnership of GPs. We serve and cover a local population of approximately 28,500 patients and are looking to recruit and welcome a Practice Nurse to enhance our team.

      We are a friendly, professional, and caring team who are passionate about providing safe and effective care. Our excellent multi-skilled Clinical Team comprises of 19 GPs (Partner & Salaried), Pharmacists, ANP, Pharmacy technicians, Practice Nurses & HCA's.

      Our team also includes a full administration team, including a reception team who work in a Patient Advisor role providing enhanced signposting and ensuring patients have access to the appropriate clinicians or services available to them.

      Supported by our excellent Clinical and non-Clinical teams, Ashfields Primary Care Centre is a consistently high performer in relation to its contractual obligations including QOF and Quality Contract services.

      As an established Training Practice, we pride ourselves in our commitment to teaching and support of a variety of clinical educational placements.

      Job description

      Job responsibilities

      The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

      1. Support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager
      1. Support the production of the IPC annual statement in conjunction with the practice manager
      1. To develop, implement and embed an effective care management programfor the frail and housebound patients registered at the practice
      1. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
      1. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
      1. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
      1. As required, to provide routine nursing care to patients in accordance with clinical based evidence and NICE guidelines
      1. To prescribe medication for patients in accordance with PGDs and own scope of practice
      1. To review patient medications to enhance compliance
      1. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
      1. To co-ordinate the provision of travel medicine services
      1. To request pathology services and process pathology results as required
      1. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
