      Job summary

      The main aims of this post are to undertake appropriate nursing care of patients attending the surgery, to contribute to the monitoring and management of patients with chronic disease, and to provide health promotion advice and support to the practice population.

      Main duties of the job

      Clinical Care

      To undertake relevant procedures as appropriate to patient need, including (but not exclusively):

      Special interest in diabetes

      Venepuncture

      Wound management and dressings

      Assisting with minor operations

      Undertaking cervical smears

      Ear syringing

      Administering immunisations (including childhood immunisations) and other injections

      Blood pressure monitoring

      Management of selected chronic conditions according to templates and protocols

      Providing effective health promotion advice and education to patients and their families including routine well-person and new patient health checks

      Giving contraceptive advice including assisting with IUCD fitting

      Advising on the appropriate management of minor illnesses

      Providing advice and immunisations for patients intending to travel abroad

      Undertaking ECGs and other investigations

      Participate in research monitoring and activities

      To arrange follow-up of individual patients as dictated by clinical need, either in person or by telephone.

      To seek advice on the management of patients from other clinical staff as required.

      The above duties will only be carried out once appropriate training and competence has been achieved.

      We are a CQC rated outstanding practice spread across two sites and covering 16,000 patients. We are a very research- active practice.

      We are a friendly team of 8 partners and 5 salaried GPs with a large primary care team of 1 practice manager and one systems and research manager, 1 office site manager, reception, administrative and secretarial teams, large nursing team including practice nurses, psychiatric nurse, pharmacy team, physiotherapist, social prescriber, care coordinator, and phlebotomist.

      We are a training practice having registrars and medical students in all stages of their training. We actively encourage all our staff to develop professionally.

      Job description

      Job responsibilities

      The post holder will work autonomously alongside other nursing staff to ensure adequate primary care nursing cover within the practice at all times. They will be expected to adhere to current practice policies and procedures and to other local and national guidelines as appropriate in order to provide high quality patient care.

      To maintain clear, accurate, legible and contemporaneous records all clinical encounters using the clinical computer system.

      To enter coded clinical data in accordance with the practice data entry policy, using templates when required.

      Management

      To assist in ensuring that the treatment and consulting rooms are adequately stocked with appropriate clinical equipment maintained in good working order.

      To assist in the organisation and implementation of health campaigns and programmes within the practice, including updating waiting room displays and patient information resources.

      Communication

      To communicate effectively and appropriately with patients, carers, relatives, members of the primary healthcare team, and relevant outside agencies.

      To work as part of the practice team, attending team meetings when required within contracted hours.

      Audit & Training

      To contribute to audit, research, service development, and clinical governance activities in areas relevant to the post.

      To contribute to teaching and training of medical students, GP Registrars, nursing students and other members of the primary care team as appropriate within the scope of qualifications and experience.
