      Job summary

      You will work collaboratively with the wider Practice Team to meet the needs of our patient population, following policy and procedures and working within your scope of practice.

      You will also provide support and training to junior members of the nursing team.

      Main duties of the job

      Working alongside your Practice Nurse and Nursing Team colleagues, to provide effective management of patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients.

      We are a large Practice of 15,150 patients in Sunderland North. The Practice is part of the Deep End Network delivering care to the most deprived in the area.

      Our team consists 3 GP Partners, 6 Salaried GPs, 5 ANPs, 2 PNs, 1 NA & 1 TNA, 1 HCA, 1 Phlebotomist, 1 Clinical Pharmacist, 1 Pharmacy Technician, the PCN Team and a large Admin Team.

      • The post holder is responsible for the delivery of general practice nursing to the whole practice population.
      • The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition and the provision of preventative health care to the practice population.
      • As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making in the management of patients.

      Essential

      • Must have experience in working in Primary Care.
      • Experience in chronic disease management including diabetes, heart disease and respiratory conditions.
      • Cervical sample taker.
      • Holiday vaccinations and childhood immunisations.

      Qualifications

      Essential

      • Registered General Nurse with the Nursing and Midwifery Council with a minimum of 2 years Practice Nurse experience

      Desirable

      • Nurse prescriber
