Work within clinical boundaries, demonstrating safe and competent clinical decision-making, delivering high quality clinical care and maintaining patient confidentiality at all times
- Assist and perform tasks related to patient care as directed by Lead Nurse and GPs following agreed clinical protocols
- To provide essential treatment duties such as BP checks, NHS Health checks, prescribed injections (e.g. B12 and Denosumab) BMI and lifestyle advice
- Provide holistic assessment of wounds and implement wound care including complex wound management in line with current evidence-based guidelines such as National Wound Care Strategy (Dopplers/Compression)within own level of competency

- To appropriately request and collect pathological & microbiology specimens including venepuncture, swabs, urine culture in line with local guidelines and process results as required actioning them in a timely manner in line with protocols
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health, promote patients to live healthily and encourage principles of self-care
- Provide appropriate self-care advice for patients presenting with minor illness within own level of competency and referring to GP/ANP where appropriate
- Recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines
- Perform Cardio-pulmonary Resuscitation and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines

- Provide contraception advice and sexual health guidance. Undertake annual contraceptive reviews and injections in line with practice policies

- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
- To provide general health promotion education, advice on national screening programmes and guide/refer patients to appropriate services or other health professionals as deemed appropriate
- Provide information and advice on prescribed or over-the-counter medication, on medication regimens, side effects and interactions as appropriate and within your level of competency
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support the clinical team with all safeguarding matters, in accordance with local and national policies
- Accurate recording of care provision onto patient records ensuring correct coding of all procedures and information using approved methods and templates
- Chaperoning and assisting patients where appropriate who are being examined by another clinician

Job summary
Vine Medical Group is recruiting for a Part time or Full time Practice Nurse to join our Nursing Team.
Our Practice Nurses are supported by our Lead Nurse and deliver practice nursing services within the boundaries of their scope of practice to the practice population. The Practice Nurses are responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure. They work collaboratively with our teams to meet the needs of patients and provide excellent and safe patient care. Our nursing team work across 3 of our patient-facing sites within Waterlooville.
We are looking for a motivated team player who is highly organised, enthusiastic and patient focussed with a can-do attitude and friendly personality. Training will be given if no experience in certain clinical skills (i.e. travel health).
We can consider candidates seeking 30 - 37.5 hours per week between Monday- Friday, with a working pattern to be agreed.
To ensure enhanced patient access to the practice we ask our team to support this extended hours working (Saturday mornings or early mid-week on a rotational basis).
