      Reports To: PCAN Assistant Manager

      FLSA Status: Full-time – Hourly, non-exempt as defined under Fair Labor Standards Act

      Job Summary

      Under the direction of the Primary Care Access Network (PCAN) Assistant Manager and PCAN Executive Director, the candidate will outreach, educate and facilitate the enrollment of uninsured patients into PCAN medical homes.

      Perform administrative and referral duties in interviewing individuals to determine eligibility for program assistance and link community residents with appropriate service agencies as needed.

      DISCLAIMER: This is a grant funded position. Continuation of employment depends upon grant funding, restrictions for the position, performance and/or organizational needs.

      Key Responsibilities

      Educate prospective individuals receiving care at the emergency department regarding patient-centered medical homes. This includes both outpatient and inpatient setting.

      • Assist patients with the scheduling process with a primary care provider at their preferred medical home. This includes creating the appointment (as applicable) and coordinate with PCAN partners to schedule patient appointments.
      • Provide outreach/education to patients, families, caregivers, social workers/case managers, faith-based programs, senior services, and school nurse programs in designated zip code areas in the Orange County community to include Hospitals, Neighborhood Center for Families, and Health Fairs.
      • Provide screening and assessment to determine eligibility of patients for programs through the PCAN member agencies and other community partners.
      • Provide information to patients on connecting with appropriate resources to apply for Medicaid and Medicare programs.
      • Provide information to patients on connecting with a Marketplace Navigator or Community Access Coordinator to assist with obtaining healthcare coverage.
      • Attends and actively participates in regularly scheduled workgroups to include but not limited to PCAN Case Management, school nurse meetings, health fairs, community presentations, etc.
      • Compile data on a routine basis to complete monthly reports of referrals and other key information metrics as deemed necessary by project leadership.
      • Maintain data collection logs (spreadsheets).
      • Provide monthly reports on tracking spreadsheets.
      • Attend internal and external meetings.
      • Contribute to achievement of project objectives.
      • Performs all other duties as assigned by True Health Outreach and Enrollment Manager and/or PCAN Executive Director.
      • Travel as necessary using personal vehicle (must maintain current auto insurance at own expense).

      Minimum Qualifications

      • Education:
        1. Associate’s degree or higher from an accredited college or university in human services, social sciences, social work, nursing, health education, health planning, healthcare administration, or related field, Preferred
        2. High School Diploma, GED, or equivalent work experience, Required

      • Experience:
        1. Proficiency in Microsoft Office (Ex. Word, Excel, Outlook, PowerPoint), Required
        2. Minimum of 2 years of professional experience working in the community or social services, Preferred
        3. Bilingual in English and Spanish or Creole, Preferred
        4. Previous Case Management program experience, Preferred
