Job summary
Swift Primary Care Network are looking for an experienced manager to join their PCN leadership team. Working closely with the PCN Clinical Directors, PCN Finance Manager, PCN operations manager and the PCN Practice Managers, this is a key role to aid the ongoing development of the PCN, integration into the locality and driving forward the PCN vision.
Swift PCN covers 80000 patients in South Bristol, across 7 GP practices: Armada family practice, Downton Road surgery, Hartwood Healthcare, Hillview Family Practice, Grange Road Surgery, The Lennard Surgery and Bedminster Family Practice.
The PCN Manager will play a central role in the continued development of the PCN, supporting the PCN Management team and managing services delivered through the PCN.
PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.
It is crucial that PCNs are visible in their local community and link with partner organisations from voluntary sector, social care, local authority and other health care organisations and part of this role will be dedicated to representing Swift in these forums.
Main duties of the job
Work with the PCN Clinical Directors and Management Team (see job description for further details)
Manage PCN services:
-Ensure services work smoothly with member practices meeting the expectations of stakeholders
-Ensure services meet their contractual requirements and agreed performance metrics
-Ensure all new PCN staff members and contractors, (where appropriate) receive appropriate induction and meet compliance requirements.
-Ensure for all PCN staff members (where appropriate), there is appropriate support including mentoring and appraisal to identify individual personal development plans
-Co-ordinate delivery of Enhanced Services, including contractual negotiations where needed and ensure delivery against the PCN DES.
- Ensure contractual requirements for the PCN DES are achieved
Work with the Practice Managers to:
-Ensure the PCN supports practices to deliver services efficiently ensuring the PCN Management team and staff effectively contribute to tackling the workload of practices and find ways to encourage the practices to work together for their collective benefit
Work with members of the Practices to:
-Promote Swift PCN strategic direction and brand to PCN practice teams ensuring appropriate levels of usage of services and engagement.
- Promote Swift PCN to other organisations and individuals within the PCN geography.
-Ensure use of digital technology is explored, where proven to benefit practice and patients.

Work with the PCN Clinical Directors and Management Team to:

- Identify and develop alternative models of care that will improve primary healthcare provision from both clinician and patient perspectives
- Work with the PCN practices to design new services including an agreed implementation plan and performance management metrics.
- Lead and support the PCN on delivery and implementation plans for new services to ensure they are well planned, communicated and delivered on time
- Look for opportunities for the PCN to be involved in research, projects and access funding to benefit our practices and patients.
- Lead on engagement with local organisations within the Swift/South Bristol footprint
- Present financial plans, strategic goals and monitoring information to the PCN exec group
