- Under supervision and support of GPs in the practice, the post-holder will strive to maintain quality within the practices, and will:
- 1.Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement.
- 2.Alerts other team members to concerns about risk, quality, and safety.
- 3.Participates in investigation of incidents and events as required.
- 4.Identifies, applies, and disseminates research findings relating to own practice.
- 5.Collects data for audit purposes and uses clinical audit to monitor quality in the service.
- 6.Contributes to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance.
- 7.Works effectively with individuals in other agencies to meet patients needs.
- 8.Effectively manages own time, workload, and resources.
- 9.Meets timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding.

Job summary
We are seeking a motivated, innovative and experienced Care Coordinator with proven leadership skills, in an autonomous position, to join our professional and friendly multi-disciplinary team which includes Advanced Nurse Practitioners, Clinical Pharmacists, General Practice Assistants and First Contact Physiotherapists. This is a great opportunity to work in a professional and wider multi-disciplinary team. You shall be accountable to the Board and GP Partners.
Main duties of the job
The post holder will be employed by Together First CIC on behalf of the practices in New West Primary Care Network (PCN), Barking and Dagenham.
The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service. The Care Coordinators role will support the clinical director and member practices in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patients journey through primary care.
About us
New West Primary Care Network consists of 4 GP surgeries over 5 sites in Barking and Dagenham covering a total population size of over 43,000 patients.
You will be required to work 17.5 hours per week over five days, with shifts taking place during the afternoons and evenings. This is a job share position, with the potential to become permanent. The role includes a six-month probationary period.

Job responsibilities
The Care Coordinator responsibilities include but are not limited to the following:
1. To work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients.
2. To work closely and in partnership with all other roles.
3. To support patients to utilise decision aids, help create single personalised care and support plans, in line with best practice.
4. To support the Clinical Director and member practices in the delivery of the DES specifications.
5. To help people to manage their needs through answering queries, making, and managing appointments.
6. To provide coordination and navigation with the aid of digital tools for people and their carers across health and care services.
7. To support the coordination and delivery of MDTs within the PCN.
8. To be able to work with and understand the roles of a variety of different people working within the practices across the PCN, including GPs, nurses, HCAs, social prescribing link workers, clinical pharmacists, physiotherapists physicians associate, paramedics, health and wellbeing coaches, occupational therapist podiatrists, pharmacy technicians and occupational therapists.
9. To be able to support PCN healthcare roles (as listed above) with their personalised care requirements.
10. To liaise with other care coordinators within other PCNs to share best practice.
11. To support in the delivery of enhanced services and other service requirements on behalf of the PCN.
12. To undertake all mandatory training and induction programmes.
13. To contribute to and embrace the spectrum of clinical governance.
14. To develop yourself and the role through participation in training and service redesign activities.
15. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
