Job summary
To be responsible for the delivery of general practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The primary care nurse will be responsible for a number of clinical and non-clinical areas such as infection prevention and control, health promotion including cervical cytology, chronic disease management, health promotion as well as supporting the management team in the reviewing of clinical policy and procedures.
Main duties of the job
Maintain CPD by attending study days courses appropriate to the needs of Davenal House surgery.
IImplement and evaluate individual treatment plans for chronic disease patients.
Provide chronic disease clinics, delivering patient care as necessary using evidenced based practise, referring patients to secondary or specialist care as required following discussion with NP GP PA.
Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
Provide routine nursing care and guidance to patients as required in accordance with clinical based evidence eg National Institute for Health and Care Excellence.

Job responsibilities
The following are the core responsibilities of the practice and primary care nurse team. There may be, on occasion, a requirement to conduct other tasks. This will be dependent upon factors such as workload, staffing levels and current competency:
Maintain CPD by attending study days courses appropriate to the needs of Davenal House surgery.
Implement and evaluate individual treatment plans for chronic disease patients.
Provide chronic disease clinics, delivering patient care as necessary using evidenced based practise, referring patients to secondary or specialist care as required following discussion with NP GP PA.
Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
Provide routine nursing care/guidance to patients as required in accordance with clinical based evidence eg National Institute for Health and Care Excellence.
Wound care.
Contraception.
Travel advice and vaccination.
Cervical cytology, including ongoing auditing of results and maintenance of policies and procedures.
Administration of injectable, non-vaccine medications.
Participation and maintenance of resuscitation policies/procedures.
Checking emergency equipment and drugs and consumables and ordering replacements if needed.
Phlebotomy.
ECG recordings
Maintain accurate clinical records.
Maintain chronic disease registers in conjunction with admin recall staff.
Chaperone patients where necessary
Support the team in dealing with clinical emergencies.
Implement vaccination programmes for adults and children.
Liaison with external service and agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
Ability to work as a team member and autonomously.
Effective time management (planning and organising)
Delegation of clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
Support the clinical team with all safeguarding matters in accordance with local and national policies.
Deliver opportunistic health promotion where appropriate.
Monitor and ensure the safe storage, rotation, and disposal of medicines/vaccines.
Lead/complete audits where necessary.
Stock/vaccine checks and ordering.
Maintaining and updating vaccine policies and procedures.
Maintaining and updating the cold chain including policies and procedures.
Completion of annual mandatory clinical and non-clinical updates via Blue Stream.
Participate in local initiatives to enhance service delivery and patient care.
Support and participate in shared learning within the practice.
Continually review clinical practices, responding to national policies and initiatives where appropriate.
Participate in extended hours when needed on a rota basis.
