- Medicines optimisation working closely with practice management to deliver key prescribing objectives in structured medication review

- Postgraduate clinical pharmacy diploma or equivalent experience.
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- Evidence of involvement in the development of programmes of care, protocols and audit
- Experience of working in a multidisciplinary team
- Have an understanding and working knowledge of the FP10 data management system (ePACT).
- Experience of co-ordinating complex activities / programmes of care with other professionals and agencies.
- Effective communicator able to communicate highly complex and sensitive information where there may be barriers to understanding.
- Demonstrates accountability for delivering professional expertise and direct service provision
- Able to work independently.
- Confident in the use of computer systems, spreadsheets, databases, data collection and coordination, wordprocessing and report writing
- Understanding of information governance, data protection and patient confidentiality in relation to information systems

Job summary
The Senior Pharmacist will undertake clinical medication reviews and patient facing clinics, working in partnership with the patient's GP practice, nurses, care homes, community pharmacists, secondary care and the local authority.
Utilising their prescribing qualification, the role will support the medication needs of the patients as well as educate other healthcare professionals on prescribing and the effective use of medicines.
The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the new era.
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
