      HealthRIGHT 360 is currently looking to fill a full-time Primary Care Provider position opening who would work at two of our San Mateo County program sites: the HealthRIGHT 360 San Mateo MAT Clinic in San Mateo and the Cordilleras Mental Health Rehabilitation Center primary care clinic in Redwood City!

      The ideal candidate has direct patient care experience working in a primary care setting, has excellent skills in motivational interviewing and an understanding of harm reduction and trauma-informed care, and has successfully worked with issues of substance abuse, mental health, and medical complexity. In this position you will work with unique patient populations and be part of 2 small teams providing primary care in a mental health setting and providing medications for addiction treatment in a community setting. This position offers the ability to work independently and as part of team, utilizing the full scope of your license in an integrated behavioral health/primary care model! Sound interesting?       The San Mateo Medication Assisted Treatment Program opened in 2016 and was created to fill the gap for substance use treatment within San Mateo County. We are warm, welcoming, and non-judgmental. Our patients struggle with addiction and come from varying backgrounds including homelessness and previous incarceration.

      The clinic staff collaborates with the county and their Integrated Medicated Assisted Treatment team to provide fully integrated medical and behavioral support to patients with substance use disorders. We also work with other HealthRight360 programs and outside agencies in a collaborative cross functional model to help people make meaningful changes in their lives. We also provide primary care to our patients who have struggled in more mainstream primary care settings- addressing acute, chronic and preventive care needs. This role will spend 60% of their time at the San Mateo Medication Assisted Treatment Program on a set schedule (3 days per week).

      Cordilleras Mental Health Rehabilitation Center (MHRC) houses two separate treatment programs for adults with chronic mental illness; a licensed 68-bed mental health rehabilitation center (MHRC) and a licensed 49-bed adult residential facility (ARF) called The Suites. Cordilleras primarily serves San Mateo County residents, 18 and older, with long histories of mental illness and multiple episodes of acute psychiatric hospitalization. The goal of Cordilleras is to allow individuals who would otherwise be in a state hospital or an acute care setting to develop the skills and supports needed to live more independently in the community. Cordilleras also offers treatment options for persons who have substance abuse issues in addition to a serious mental illness.

      Telecare provides care for the patients while they are on the unit, incorporating such approaches as motivational interviewing, cognitive behavioral therapy, dialectical behavioral therapy (DBT), harm reduction, and other consumer-centered therapeutic interventions in addition to medication management by a psychiatrist.

      HealthRIGHT 360 operates a primary care clinic for the residents of the rehabilitation center in a separate space on the first floor. In addition, we work closely with Telecare to ensure that all residents, whether or not they are seen in the primary care clinic, are receiving appropriate preventive care and management of their chronic illnesses. This role will spend 40% of their time at the Cordilleras Mental Health Rehabilitation Center on a set schedule (2 days per week).

      Key Responsibilities

      San Mateo MAT Clinic

      • Manages substance use disorders by offering/prescribing medications in addiction treatment (MAT) and offers transitional treatment of primary care conditions, within the scope of credentialed privileges and standardized procedures, to patients of the San Mateo MAT Clinic, utilizing trauma-informed, motivational interviewing skills and encouraging patient activation and self-management.
      • Provides or makes arrangement for preventive care and immunizations and management of patient’s chronic disease.
      • Works collaboratively with community partners and other HealthRIGHT 360 sites including IMAT team, residential and outpatient treatment programs and primary care and mental health providers.

      Cordilleras MHRC Primary Care Clinic

      • Provides comprehensive primary care, within the scope of credentialed privileges and standardized procedures, to residents of the Cordilleras MHRC seen within the primary care clinic on site, utilizing trauma-informed, motivational interviewing skills and encouraging patient activation and self-management.
      • Manages co-occurring substance use disorders by offering/prescribing medication assisted treatment (MAT).
      • Ensures appropriate chronic and preventive care for all patients residing on the unit, whether or not they have been seen by the medical team.
      • Work collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes.

      Overall

      • Ensures appropriate follow-up for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.) and medical hospitalizations and ER visits.
      • Responds to patient and community partner requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner.
      • Demonstrates comfort in changing systems, champions change and leads change processes.

      Education and Knowledge, Skills and Abilities

      Education and Knowledge, Skills and Abilities

      • Masters or doctorate degree from an accredited nursing or physician assistant school.
      • Possession of a current, valid California Registered Nurse License as an ANP or FNP and Furnishing Number or Physician Assistant License.
      • Possession of national certification from ANCC (American Nurses Credentialing Center), AANP (American.
      • Association of Nurse Practitioners), or NCCPA (National Commission on Certification of Physician Assistants).
      • DEA registration and X-waiver.
      • NPI number.
      • Experience working successfully with issues of mental illness, substance abuse and criminal backgrounds.
      • Bilingual/fluent in English/Spanish preferred.
      • Direct patient care experience in a ambulatory care setting and/or a community health center, preferred.
      • Knowledge of co-occurring disorders and trauma-informed care.
      • Understanding of harm reduction model and its role in treatment for substance use disorders.
      • Excellent verbal, written and interpersonal skills.
      • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.
      • Strong initiative and enthusiasm and willingness to pitch in whenever needed.
      • Experience working with electronic health records (E-clinical works, preferred).
      • Demonstrates comfort in changing systems, champions change and leads change processes.

      Salary/Benefits/Schedule

      Salary is commensurate with experience. HealthRIGHT 360 offers a comprehensive benefits package that includes medical, dental, chiropractic, acupuncture, vision, paid holidays, PTO, and more! Hours are Monday – Friday; some evenings required.
