      Job summary

      St Pauls Primary Care Network presents an exciting opportunity for an experiencedUrgent Care Practitioner or Advanced Nurse Practitionerto work across the four practices that form the Network, all housed in St Pauls Medical Centre in the heart of Cheltenham.

      The successful candidate will work across the four practices continuing to support the successful home visiting service, and in addition supporting the PCN and Practices with the provision of Urgent Care to patients. This new and exciting role will be closely supported by the GPs, Allied Health Professionals and a team of Clinical Pharmacists, Pharmacy Technicians and Social Prescribers.

      More specifically, our Urgent Care Practitioners will be primarily responsible for both acute and routine Home Visiting as well as triage, support and diagnosis of patients with same-day queries.

      We are offering a part time position of 18 hours a week, working over two-days to include a Friday.

      You'll have the chance to experience continuity of care and see cases develop from initial triage through to resolution. With the support of the other Urgent Care Practitioners across the group and the experienced clinical team on site, you will be able to share information, learn and grow in your confidence and ability.

      The post is open to both Registered Experienced Paramedics and Advanced Nurse Practitioners.

      Main duties of the job

      Key responsibilities

      Diagnosing and treating patients presenting with minor illness

      • Provide appropriate diagnosis and treatment, on routine and urgent home visits to patients, within their own home or another appropriate setting.
      • When not visiting patients at home, to provide triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team.
      • Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.
      • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary.
      • Work from the surgery and within communities as an autonomous practitioner caring for patients and families.
      • To undertake assessment of patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate.
      • Prescribe/issue medications as appropriate following policy, patient group directives and local pathways

      St Pauls Primary Care Network is made up of four practices that form the Network, all housed in St Pauls Medical Centre in the heart of Cheltenham (Weston House, St Catherines Surgery, St Georges Surgery, and the Royal Well Surgery).We have a fantasic team of Allied health professionals including paramedics, ANP's, social prescribers, health care assistants, FCP, pharmacists and pharmacy technicians.

      • Registered with the HCPC and/or NMC
      • DipHE/FDg in Paramedic Sciences or equivalent healthcare qualification
      • Level 5 or 6 HEI module in Minor Injury/Illness or equivalent

      Desirable

      • Non-medical prescribing qualification

      Experience

      Essential

      • Experience of working to protocols, guidelines and Patient Group Directives
      • 2 years post registration experience

      Desirable

      • Knowledge/experience of clinical medical systems
      • Knowledge/experience of SystmOne
      • Experience of working in General Practice
      • 2 years experience of working as an emergency care practitioner in General Practice or A&E
