      Essential Job Responsibilities:

      1. Knowledge of Mission Statement, Employee Promise, Philosophy of Care, Customer Service Foundations
      2. Provide and direct resident care in accordance with SNF/ICF guidelines, HSS-132 Nursing Home Rules and Regulations, Resident Bill of Rights, Nursing Services Policies and Procedures, and the Homestead Village Employee Policy Handbook.
      3. Observe, assess and report changes in residents’ condition to immediate supervisor, attending physician, family or designated contact person.
      4. Call or accept verbal orders from resident’s physician and transcribe them according to procedures and policies. Orders, both verbal and written, are to be transcribed timely and accurately.
      5. Actively coach caregivers to ensure outstanding resident care; utilize coaching supervision training and techniques.
      6. Work cooperatively as part of a self-directed work team, utilizing 3P skills, consensus decision making and treating team members with respect.
      7. Communicates readily with team members and RN nurse leaders.
      8. Administer medications and treatments according to physician orders. Document appropriately and according to Med A guidelines if necessary and report any drug and/or treatment errors to supervisor and physician.
      9. Initiate and update care plans and problem lists at all times.
      10. Serve as role model in the implementation of Homestead Village policy and OSHA standard on Universal precautions ensuring staff compliance and follow-up with required retraining and disciplinary action.
      11. Documents in resident charts and maintains clinical records in prescribed manner.
      12. Reports staff and/or resident concerns/abuse immediately to supervisor.
      13. Complete skin assessments documenting any findings, per policy.
      14. Completes incident reports in their entirety as accurately and timely as possible.
      15. Review daily reports from assigned personnel and attend weekly care plan meetings relative to residents total plan of care and communicate to oncoming shift.
      16. Operate equipment required for resident care and provide life saving care, i.e.: suctioning, etc.
      17. May assist physician during examination and/or treatment of residents; review orders with physician, suggest changes based on resident’s condition.
      18. Participate in the development of policies and procedures to improve nursing care programs.
      19. Assist in training, supervising, counseling and evaluating assigned staff. Replace absent staff when needed.
      20. Attend scheduled meetings, continuing education programs and mandatory in-services as an active participant of Nursing team to keep abreast of current trends in medical science with special emphasis in the area of gerontology.
      21. Be flexible, receptive and adaptive to change.
      22. Replace absent staff as necessary.
      23. Participate in orientation of new employees.
      24. May attend care plan meetings.
      25. Maintain and safeguard entrusted confidential information.
      26. Understand and apply basic principles of good housekeeping and safety.
      27. Adjust assignments and schedules to best meet needs of residents with appropriate personnel.
      28. Pass along pertinent information needed for 24 hour report.
      29. Must be knowledgeable of emergency procedures in the event of a fire, tornado, bomb, etc. and able to carry through the correct procedure should an emergent situation arise.
      30. Respond to apartment emergencies, as needed.
      31. Perform other duties as assigned.
