- GCSEs Maths and English (C and above) or equivalent.

Job summary
Proactive Care is an NHS Long Term Plan that aims to provide proactive and personalised support for a targeted subset of individuals living with multiple long-term conditions. This service supports adult patients and carers with complex needs who could benefit from proactive Multi-Disciplinary Teams (MDTs) in local communities to be able to live independently for longer and to reduce avoidable hospital admissions and GP visits.
The Proactive Care Assistant will have Administration Assistant responsibilities. This is an evolving and exciting role and therefore the post holder should demonstrate a flexible and positive attitude, to change with the needs of the service and service users.
The Proactive Care Assistant will proactively case find and identify patients who are most vulnerable from admission and/or health deterioration and work closely with the therapy team to support patients to manage their health conditions and to achieve the best quality of life and independence possible through robust health assessments and care planning.
Main duties of the job
Be responsible for daily proactive case finding of patients on EMIS and Connected Care to identify appropriate patients for a proactive care assessment.
To participate in the smooth running of the proactive care service by undertaking administrative tasks e.g. making appointments, telephone calls and other clerical activities.
Be responsible for arranging, attending and taking minutes for Multi-Disciplinary Team meetings.
Be responsible for completion of actions following MDT which may include generating letters and tasks to patients GP.
To maintain high quality, contemporaneous records and assist the therapists and practitioners as necessary with data collection and analysis.
To be able to use good verbal and non-verbal communication skills with patients and relatives, carers and colleagues including those with communication difficulties, adapting communication as required to meet the needs of the patient or carer.
Dealing with patients, relatives and colleagues queries both in person and on the telephone confidentially, courteously and efficiently.
Be responsible for logging and making referrals.
Confident IT skills, to include Microsoft Office, Outlook and Excel.
To support seasonal projects as requested by a manager.
To carry out any other reasonable duties as requested by a manager to ensure the quality of the service.
