What to Expect (Job Responsibilities):
- Develop and maintain primary program tracker within excel and monday.com
- Perform quantitative analysis to support program operations
- Conduct research and analysis to identify and test opportunities for program expansion
- Support post-acute transition specialists in facilitating SNF communications and in-person visits
- Assist with practice engagement workstreams by scheduling/facilitating calls with practices

What is Required (Qualifications):
- Bachelor's Degree
- 2+ years of professional experience
- Proficient in Microsoft Excel and Monday.com
- Experience working with cross-functional teams
- Ability to work in a fast-paced environment and manage multiple priorities
