Directs personnel, operational and financial activities over multiple sites, inclusive of clinical staff working under community health programs
Identify and implement programs and services to support vulnerable populations through wrap around social and clinical care integration
Oversee staffing and teambuilding activities-- develop, coach, mentor and motivate colleagues
Ensure compliance with established federal guidelines, Hospital, personnel policies when operating the safety net clinics
Develop goals, objectives, plans and budgets for all programs within CHWB to ensure continued quality of services through planning across the region with other functional areas
Oversee measurable community health improvement plans and implementation strategies that align with the community health needs assessment and Trinity Health strategic plan
Identify funding sources such as foundations, state and federal agencies and establish reporting systems for each funded initiative
Represent Ministry at professional or community organization meetings and on committees at local, state and national level in order to assist in developing new programs and policies
Assure that required program documentation is maintained for state and federal grant compliance
Bachelor's degree in Health Care Administration, Public Health, Community Health, Social Work or a related field. Master’s Degree is preferred
Strong knowledge of clinical operations and quality improvement processes
Seven (7) years of community health and well-being experience with demonstrated progressive responsibilities in strategic planning, fiscal management and operations
Five (5) years of experience in a team or initiative leadership capacity is required
- Full time 40hrs M-F days

Based out of Saint Francis, will have oversight of Community Outreach for Mercy Medical Center in Springfield, MAPosition Purpose
Ensures that the region is fulfilling its commitment to promote optimal health for those communities we serve by connecting social and clinical care, addressing social needs, dismantling systemic racism and reducing health inequities. This position sets strategy, leverages resources and partnerships focusing on those who are poor and vulnerable, and addressing the social influencers of health. This position provides oversight of direct service community health programs operated in safety net clinics and clinical personnel staffing programs, and acts as a liaison to community agencies in determining community needs. In addition, this role requires strong collaboration with community-based organizations, city officials and state agencies.
