      POSITION SUMMARY:

      The Care Coordination Redesign Program (CCR) is a grant-funded program through the New York City Department of Health and Mental Hygiene. CCR is a medical case management program that supports the retention in care and treatment adherence for people living with HIV (PLWH) who are historically susceptible to falling out of care. The program supports home and field-based patient navigation services, coordinated medical and social services, counseling and coaching in adherence to treatment and health education to build the skills and knowledge needed to maintain stable health. The program seeks to address health disparities by facilitating access to care and social services through a client-centered, holistic, and comprehensive approach to meet the needs of enrolled patients. The CCR Program Manager provides leadership and oversight for the CCR Program. S/he acts as a role model for all Care Coordination personnel in the planning, implementation and delivery of client services. With the support of the Director of Programs, the CCR Program Manager ensures both internal and external programmatic compliance; collaborating across departments to develop, deliver and evaluate the program deliverables in accordance with New York State Department of Health and Mental Hygiene and Public Health Solutions as well as agency policies, procedures and quality standards.

      RESPONSIBILITIES:

      Monitors compliance of CCR Program grant deliverables and activities, ensuring program goals are met

      Serve as program’s external liaison, supporting the timely completion of grant reporting

      Responsible for the recruitment, training and administrative supervision of program

      Conduct performance management activities, including but not limited to appraisals, documentation, coaching and disciplinary actions for program staff at the program at JPAFHC

      Ensures that Care Coordination services provided meet or exceed service targets

      Conducts quality assurance reviews to monitor and audit compliance with indicators and program metrics. Ensuring department standards of practice are met and implement plans of correction when necessary to address substandard performance.

      Support the program preparation for all internal and external audits

      Direct, coordinate, and implement training programs for Care Coordination staff, including both internal training and outsourced instruction

      Build, develop, and maintain community relationships, linkage agreements and referral arrangements with community agencies and hospitals

      Participate in intra- and inter-agency community meetings and committees as assigned

      Analyze program performance and forecasts future demand and resource requirements for continued effective program operation.

      Provide counseling and crisis intervention support for patients and staff as needed.

      Attend and participates in continuing education programs, seminars designed to maintain professional competence.

      Assure compliance with all policies, regulations and laws governing clients’ rights and confidentiality of information.

      Performs other duties as required by the Program Director, Care Coordination and Clinical Outcomes or his/her designee


      Minimum Qualifications:

      1. Bachelor’s Degree, MSW/ MPH or related Master’s Degree preferred

      2. Three (3) years of experience working with the HIV priority populations, social services and/or community outreach settings. Experience with Care Coordination for PLHW preferred.

      3. Computer literacy with proficiency in MS Word, Excel and PowerPoint as well as electronic medical records.

      4. Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time

      5. Ability to generate, interpret and analyze data from multiple sources

      6. Technical knowledge of program development and relevant community resources, ability to develop linkage with community organizations

      7. Experience administering grant-based contracts preferred

      8. Must have cultural competency and comfort working with LGBTQ populations.

      9. Bi-lingual (English/Spanish) preferred.
