- Bachelors degree in Community Health, Business or Healthcare related field
- 3 years healthcare experience is required
- Someone with a background or interest in health, wellness, and chronic disease management with a strong aptitude for relationship building
- A strong collaborator with the willingness and ability to partner with our Total rewards team to promote our diabetes management programs

- This position will include monthly travel within our health system (Minnesota, North Dakota, Wisconsin)
- Candidates must sit within our service area and be able to come onsite
- Most of this work will be conducted remotely

1006090 POPULATION CARE MANAGEMENT - EH SSJob Description:
Responsible to provide leadership, program development and coordination between providers, staff and patients for a defined population. Acts as a primary liaison between the operations team, internal stakeholders and key community partners. In conjunction with internal departments and disciplines, coordinates community outreach programs, including screening and health promotion activities, and education. Ongoing collaboration with providers, staff, quality, regulatory and reimbursing agencies are required for optimal care coordination across the continuum. Position requires knowledge and ability to research evidence-based practices analyze and interpret data to make program recommendations and improve population outcomes.Education Qualifications:
